The United Kingdom is currently experiencing an E.coli outbreak, with over a hundred confirmed cases.

Out of these, there have been at least 37 hospital admissions and the UK Health Security Agency (UKHSA) suggests that the number of cases may continue to escalate.

The age range of confirmed infections spans from two to 79 years, with a higher incidence amongst young adults.

But what exactly is E.coli, how can it be identified, and what are its symptoms?

Understanding E.coli

Escherichia coli, or E.coli, is a bacterium typically found in the guts of warm-blooded organisms, as explained by the World Health Organisation (WHO).

Not all strains of E.coli are harmful, but there are some, like the Shiga toxin-producing E.coli (Stec), that can lead to severe foodborne illnesses.

This particular outbreak has been associated with the Stec type of the bacterium.

Annually, the UK sees approximately 1,500 cases of Stec infections, reports the UKHSA.

Transmission of E.coli

Stec is commonly linked to the consumption of raw or undercooked ground meat products, unpasteurized milk, and contaminated vegetables, as WHO indicates.

The infection can be foodborne but can also spread through direct contact with an infected person or animal and their environments.

The UKHSA has excluded waterborne transmission in this scenario, clarifying there is “no evidence connecting the outbreak with open farms, drinking water, or swimming in contaminated waters”.

The source of this outbreak is presumed to be a “nationally distributed food product“, per the UKHSA, although the specific item has not been disclosed.

Recognizing Symptoms of Stec

Stomach cramps, severe diarrhea (may be bloody), vomiting, and fever are among the primary symptoms of a Stec infection, as outlined by the UKHSA.

While some infected individuals may experience only mild diarrhea or no symptoms, others may endure symptoms lasting up to two weeks.

In more acute cases, particularly in children, Stec can lead to haemolytic uraemic syndrome (HUS), a serious complication that can result in kidney failure.

The UKHSA informs that HUS could develop up to two weeks after initial diarrhea symptoms begin.

Additional symptoms such as unusual bruising, decreased urine output, or discolored urine necessitate immediate medical attention, as they could signify the onset of HUS.

There is also a small chance of adults contracting thrombotic thrombocytopenic purpura (TTP), a disorder similar to HUS.

Treatment Approaches

Typical Stec infections resolve within a week without specific medical intervention. Staying hydrated is crucial to combat dehydration from diarrhea.

Contrarily, the use of antibiotics is discouraged as they might increase the risk of complications like HUS, says the UKHSA.

E.coli Preventive Tips

The UKHSA recommends measures such as:

• Rigorous handwashing with warm water and soap since alcohol gels are ineffective against certain diarrheal pathogens
• Adhering to food safety practices like washing produce and cooking food thoroughly
• If experiencing diarrhoea or vomiting, refrain from food handling and avoid contact with healthcare or residential facilities to prevent infection spread
• Stay away from work, school, or nursery for at least 48 hours post-symptom resolution.

Previous Outbreaks in the UK

An outbreak in June of the previous year accounted for 27 Stec cases, with minced beef products implicated as the source.

In 2022, from August to October end, more than 259 illnesses were traced back to an outbreak likely caused by a certain salad.

Occasionally, recalls of products like five cheese products in December occur over outbreak concerns.

E.coli Concerns in the US

The US has had its share of E.coli-related issues, notably with romaine lettuce in 2018, affecting 22 states.

Produce from Yuma, Arizona, was suspected, leading authorities to advise on sourcing lettuce from different regions or confirming its safety.

A 2022 US Food and Drug Administration report highlighted potential storage issues that might have allowed E.coli to survive longer on romaine lettuce.

Frequently Asked Questions

What is E.coli?

Escherichia coli (E.coli) is a bacterium commonly found in the intestines of humans and animals. While most strains are harmless, certain types like the Shiga toxin-producing E.coli (Stec) can lead to severe foodborne illness.

How can E.coli be transmitted?

E.coli can be transmitted through consuming contaminated food or drink, close contact with an infected individual, or contact with an infected animal or its environment.

What are the symptoms of an E.coli infection?

The symptoms can include stomach cramps, severe and sometimes bloody diarrhea, vomiting, and fever. The severity of the symptoms can range from mild to very serious.

How is E.coli treated?

E.coli infections usually resolve without specific treatment other than rest and hydration. Antibiotics are generally not recommended as they may increase the risk of complications.

How can I prevent getting an E.coli infection?

Preventive measures include thorough hand washing with soap and water, proper food handling and preparation, cooking meats to safe temperatures, washing fruits and vegetables, and avoiding the consumption of raw milk and other unpasteurized dairy products.
